How do I submit a claim?
Submitting a claim under the AKC Pet Healthcare Plan is simple and straightforward.
- You decide which veterinarian you want to use and the treatments your pet needs.
- You pay your veterinarian in the normal way and submit your claim for reimbursement.
- You will receive a claims explanation of benefits detailing how your claim was processed.
A few important notes before submitting your claim for reimbursement.
- Claims cannot be processed without original itemized receipts for veterinary treatment.
Please make sure you enclose these with your claim form (keep copies for your records).
- Send your completed claim form within 180 days of the date of service for you pet's treatment.
- The policy contains certain restrictions on coverage. Please make sure you read your Policy Terms
