You can also call Customer Service at 1.866.725.2747 and request that a claim form be mailed to you.

How do I submit a claim?

Submitting a claim under the AKC Pet Healthcare Plan is simple and straightforward.

You decide which veterinarian you want to use and the treatments your pet needs.
You pay your veterinarian in the normal way and submit your claim for reimbursement.
You will receive a claims explanation of benefits detailing how your claim was processed.

A few important notes before submitting your claim for reimbursement.

Claims cannot be processed without original itemized receipts for veterinary treatment. Please make sure you enclose these with your claim form (keep copies for your records).
Send your completed claim form within 180 days of the date of service for you pet's treatment.
The policy contains certain restrictions on coverage. Please make sure you read your Policy Terms and Conditions and are aware of your benefit coverage. If you have any additional questions please contact us.
